La fluorose à Dakar: un cas de Compression Médullaire Lente cervicale observé à la Clinique Neurologique du CHU de Fann, Dakar-Sénégal

Résumé La fluorose osseuse résulte de l’accumulation progressive de fluor dans le tissu osseux. Elle est habituellement asymptomatique au début et ne se voit qu’à l’âge adulte car elle nécessite de longues années d’exposition à des doses de fluor très élevées. L’intoxication survient exclusivement par ingestion du fluor. La source principale de contamination est l’eau de boisson. Elle est devenue fréquente mais grave du fait de ses complications neurologiques souvent invalidantes voire mortel es. Nous rapportons le cas de B D, 38 ans, sexe masculin, sénégalais habitant dans la région de Thiès, hospitalisé en Avril 2007 à la Clinique Neurologique du CHU de Fann, Dakar-Sénégal pour un syndrome de compression médullaire lente cervicale. Le bilan radiologique et biologique a confirmé une origine liée à une fluorose osseuse : Fluorémie très élevée : 293,80 µg/l, Fluorurie très élevée : 19194,8 µg/l, ostéocondensation diffuse du rachis cervico-dorso-lombosacré. La tomodensitométrie cervicale et l’imagerie par résonnance magnétique ont confirmé le rétrécissement cervical et la calcification du ligament vertébral. Summary Fluorosis in Dakar: a case of spinal cord compression observed at the Department of Neurology, Fann University Teaching Hospital, Dakar-Senegal. The bone fluorosis results from the progressive accumulation of fluorine in bone tissue. It is usually asymptomatic at the beginning however symptoms and signs are revealed at the adulthood after several years of exposure to very high dose of fluorine. The intoxication occurs exclusively by ingestion of fluorine. The primary source of contamination is the drink water. It is becoming frequent but serious because of neurological complications often invalidating even lethal. We report the case of B D, a 38 year Senegalese man living in a village located in the region of Thiès, hospitalized in April 2007 at the Department of Neurology, Fann University Teaching Hospital, Dakar-Senegal for a spinal cord compression syndrom at the cervical level. The radiological and biological assessment confirmed a bone-fluorosis related cause: Very high blood (293.80 µg/l) and urine (19194.8 µg/l) Fluorine concentration, generalized osteocondensation of the backbone. A CT scan of the cervical neck confirmed the spinal cord compression associated with the calcification of the vertebral ligament. The evolution was lethal with death two days after neurosurgical intervention.
